Student Physical Examination and Immunization Records (HRS 3040 and HRS 680, also known as "Blue and Gold Forms") Parents should comply with diocesan immunization policies. For more information please click here.

Students entering VPK must be four years old by September 1st
Students entering Kindergarten must be five years old by September 1st
Comittment to complete 30 hours of service in support of the school each year
Students will be conditionally accepted into Kindergarten with full acceptance contingent on displayed readiness.
Students applying for Admission in Grades 1-8 must present a copy of the current report card and standardized test results. These will be reviewed to determine whether the program at Saint Anthony Catholic School will meet the educational needs of the students.
